In a word - youre basically fighting them both, but fortunately Miquella doesnt have a separate health bar.
Oddly enough, the only moment of refuge is if you happen to be grabbed by Radahn.

Its a strange moment, but well take any respite we can.
If you have a Spirit Ash, you ideally dont want them vanquished halfway through the fight either.
With a little luck, this otherwise nightmarish fight can be over much quicker than you think.
